Practitioners and facilities can file official responses to reports on Malpracticed.

Option 1: Enter Report Hash

Option 2: Search by Name or License

How It Works

  1. Find the report you want to respond to (by hash or practitioner name)
  2. Select the specific violation
  3. Provide your identity verification and malpractice insurance / medical license
  4. Write your official response
  5. Your response is hash-chain ledgered and displayed alongside the report
